A WATCH will monitor all new entries to the system for person's whose data matches the data you enter here. If there is a close match between the records, you will be notified of that fact by email, and optionally by your text messaging capable pager or cell phone.

This record will not be available to the general public, but will be available to emergency & medical personnel. If needed, they may contact you via telephone or direct visit if additional information is required.

Please complete all fields to the best of your knowledge, including as much information as is known about the individual. Identification information, and uploading a picture of the person sought help rescue and medial personnel to identify victims that may be dazed or confused.

Medical information, including pre-existing conditions and medications currently being taken are important pieces of information for medical personnel that may treat the victim. Please include any such known information.

If posible, the person whose information is being entered will be notified that they are being sought, and by whom as soon as they are located.

These can be extremely helpful in aiding medical and rescue workers make identifications, especially of dazed or unconscious persons. Please note any previous medical conditions of the person so that medical personnel using this system can be advised. Note any medical conditions, medicines being taken, handicaps, etc.
